Affirmed Home Care is expanding to serve clients in the Hartford, CT region, and we are seeking dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide exceptional, one-on-one care in the comfort of our clients’ homes.

Competitive Compensation: $38–$45 per hour, based on license type and experience (LPN'S)
Competitive Compensation: $55–$65 per hour, based on license type and experience (RN'S)

• Conduct thorough client assessments and implement individualized care plans.

• Administer medications, treatments, and nursing procedures in accordance with physician orders.

• Monitor, evaluate, and document client conditions, communicating updates promptly to care teams.

• Deliver advanced nursing interventions, including wound care, IV therapy, and catheter management (RNs).

• Educate clients and families about health maintenance, medication compliance, and home safety.

• Maintain accurate, detailed documentation following regulatory and agency standards.

• Ensure compliance with all state, federal, and company healthcare protocols.

• Provide compassionate, patient-centered care that reflects Affirmed Home Care’s values of dignity and excellence.

• Active RN or LPN License in the State of Connecticut (required)

• Minimum 1–2 years of nursing experience (home care, geriatric, or related field preferred)

• Strong assessment and clinical decision-making skills

• Excellent communication, time management, and interpersonal abilities

• CPR/BLS certification (required)

• Reliable transportation and valid driver’s license

• Experience with Electronic Health Records (EHR) preferred
