About the Role

The Qualified Professional (QP) plays a vital role in coordinating care, providing direct interventions, and developing individualized service plans for individuals with mental health and/or substance use needs. This position works closely with the Clinical Director or a designated supervisor to ensure services meet Medicaid standards and reflect best practices in behavioral health.

What You’ll Do

As a QP with New Phoenix, LLC, you will:

• Develop, update, and maintain Person‑Centered Plans (PCPs)

• Provide direct interventions, skill‑building, and supportive services

• Coordinate referrals, community resources, and care transitions

• Ensure timely, accurate, and compliant documentation

• Participate in treatment team meetings, clinical reviews, and supervision

• Monitor individual progress and adjust service strategies as needed

Your work will directly support individuals in achieving their goals and improving their quality of life.

What We’re Looking For

To be successful in this role, candidates must:

• Meet North Carolina Qualified Professional (QP) standards for MH/SUD

• Hold a Bachelor’s or Master’s degree in a human services field

• Have experience with treatment planning and clinical documentation

• Understand Medicaid service delivery requirements

• Bring strong communication, organization, and teamwork skills

• Demonstrate compassion, professionalism, and a commitment to person‑centered care
