Client Care Coordinator - Front Range

Reports to: Clinical Director

Job Category: Hourly | Non-Exempt | Full-Time

Salary Range: $22.40-$25.76 per hour

Job Site: Foundry Front Range (Broomfield, CO)

Job Summary:

The Client Care Coordinator is responsible for the ongoing care of clients in treatment and developing continuing care plans and an effective post-treatment strategy in order to facilitate a smooth and safe transition for clients.

Education and Experience:

  • Bachelor’s degree preferred, high school diploma or equivalent required.
  • Two or more years proven experience with people and program management in clinical settings.
  • Two years experience in substance abuse or mental health field preferred.
  • Experience with EMR system preferred.

Required Skills/Abilities:

  • CPR certification (must obtain during first 30 days).
  • Strong relationship-building, communication, and active-listening skills.
  • Ability to work effectively within a multidisciplinary team.
  • Organized with strong time-management and documentation skills.
  • Experience facilitating groups and leading client activities.
  • Familiarity with 12-Step principles; trauma-informed care preferred.
  • Ability to motivate and support clients in recovery.
  • Professionalism, integrity, and strict confidentiality.
  • Strong judgment, problem-solving, and collaboration skills.
  • Knowledge of regulatory standards and guidelines, ex. HIPPA
  • Familiar with Microsoft Office, Google Workspace, or similar platforms
  • Valid, non-restricted Colorado driver’s license.

Duties/Responsibilities:

  • Provide ongoing client support, assess needs, and identify barriers to care.
  • Determine appropriate post-treatment programs considering clinical, financial, and family factors.
  • Develop and communicate continuing care plans with clients and families.
  • Provide or obtain clinical recommendations for discharge.
  • Maintain knowledge of treatment structure and transitional resources.
  • Schedule discharge logistics, appointments, transportation, and send discharge information.
  • Manage client schedules and coordinate internal/external appointments.
  • Collaborate with Admissions, Medical, Wellness, Family, and Clinical teams.
  • Maintain consistent communication with clients, families, and the clinical team.
  • Ensure accurate, timely documentation in compliance with regulatory standards.
  • Ensure client safety, satisfaction, and adherence to policies and procedures.
  • Identify and respond appropriately to emergencies.
  • Coordinate with continuing care facilities, verify benefits, and track insurance review dates.
  • Maintain confidentiality.
  • Represent organizational mission and values with professional conduct.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working/typing on a computer.
  • Sit, stand, walk, bend, reach, climb steps, hear, see, speak.
  • Must be able to lift 15 pounds at times.
  • Driving in all weather conditions.
