Position Summary:
The Parent-Child Assistance Program (PCAP) is a 3-year intervention for pregnant and parenting mothers with substance use disorders and their families. This position will be responsible for providing direct outreach, home visitation, case management and advocacy services to mothers who have engaged in at-risk alcohol or drug use during pregnancy, and to their children, partners, and extended families when needed.

RESPONSIBLITIES
Foster effective collaboration with community organizations.Perform case management activities according to PCAP policies and procedures, in a highly confidential and professional mannerAssist clients in setting goals based on their individual needs assessments, and in identifying steps to achieve their goals; monitor progress.Forge partnerships and collaborate closely with organizations offering services relevant to clients' needs.Provide outreach services to connect clients with community organizations supporting their goals and needs.Offer case management support to 15-20 clients and their families according to PCAP policies and procedures and maintain required documentation, all in a highly confidential and professional manner.Participate in regular supervisory meetings with the clinical director to address clients' needs, goals, and concerns.Actively participate in team and supervisory meetings to evaluate clients' progress and program effectiveness.Deploy strategies to re-engage clients who may have become unresponsive or unavailable, ensuring their continued participation in the program. Administer clinical assessment instruments dealing with highly sensitive and confidential information, according to PCAP policies and procedures.Collect client-related data and input into REDCap, an online data entry platform.Coordinate scheduling and attend case consultations and appointments with community providers with whom client is involved. Maintain regular contact with client’s network of service providers.Participate in required pre-service and ongoing training. Contribute to report writing, trainings, and community presentations as needed.Other duties as assigned.QUALIFICATIONS AND REQUIREMENTS
Education:
Bachelor's Degree in Social Work or related field requiredExperience:
4 years or more experience on issues/projects working with parental substance abuse or equivalent combination of education and work experience as well as experience working with at-risk populations. Life experience dealing with substance use issues or other relevant issues is preferred.Licenses/Certificates/Registration:
Obtain and maintain current CPR, Infant CPR, and First Aid Certification.Valid drivers license and vehicle insuranceWORKING CONDITIONS
