$33.04 - $49.57 an hour - Full-Time Days, 40 hours per week
Boulder Community Health is seeking a Clinic RN who will be responsible for managing the Outpatient Parenteral Antimicrobial Therapy (OPAT) program and supporting patient triage for both new and established patients under the supervision of infectious disease physicians and the nurse practitioner. This role involves coordinating individualized infectious disease care plans, collaborating with clinical staff to develop and maintain discharge and home health plans, and ensuring seamless follow-up with home health agencies, infusion services, and skilled nursing facilities. The Clinic RN will serve as a key communicator with patients, families, providers, and payers, while also playing an active role in patient education, training, and monitoring for those with complex medical needs.

Qualifications
•Colorado RN license in good standing.
•BLS certification required by the end of conditional period.
•Previous nursing and medical experience preferred.
Responsibilities
•Electronic Medical Record auditing including, but not limited to, order management, tasking and messaging, appointment recall, and inbound fax management to ensure efficiency and patient safety.
•Telephone triage calls for triage/referral in consultation with appropriate clinic provider(s).
•Retrieves and responds to phone or other messages in a timely manner. Documents all incoming and outgoing communication.
•Serves as a resource to clinic staff regarding medical information, patient assessment, and nursing management of illness.
•Occasionally assist medical assistants with routine patient care and patient notification as needed.
•Set-up outpatient infusion and home health services, and/or other discharge related care as necessary.
•Works with physicians, nurses, and patients to assess the urgency of patient problems. Coordinate scheduled appointments based on need in consultation with the appropriate clinic provider(s).
•Assesses and provide individual status and care/instructions needed by the patient as indicated.
•Consistent and on time attendance is an essential function of the position.
•Monitor labs for home health patients and notify provider when outside expected limits.
•Performs select nursing duties for patient ages starting at 15 years to geriatric, as well as select administrative duties.
•Provides patient education and instruction as necessary.
•Maintains patient files, records, and other information.
