LPN Hospice Admissions Coordinator Roswell at Blue Summit Hospice and Palliative Care – Roswell, Georgia

Blue Summit Hospice & Palliative Care is looking for an experienced hospice LPN to join our office team in McDonough/Snellville.

This is a full time in office position. Homecare Homebase experience is required.

Responsibilities:
- Enter referrals
- Answer phones and direct clinical traffic
- Visit patients if needed
- Assist Director of Nursing with EMR management and documentation
- Run reports
- Manage data

Skills:

- Hospice knowledge and experience
- EMR experience
- Office Management experience
