About the Role:

The Spanish Speaking Case Manager/Paraprofessional plays a critical role in supporting individuals and families by providing culturally and linguistically appropriate case management services. This position focuses on assessing client needs, developing individualized service plans, and coordinating access to community resources to promote client well-being and self-sufficiency. The role requires effective communication with Spanish-speaking clients to ensure clear understanding and trust, facilitating engagement and positive outcomes. The Case Manager/Paraprofessional will collaborate with multidisciplinary teams and external agencies to advocate for clients and monitor progress toward goals. Ultimately, this position aims to empower clients through comprehensive support and resource linkage, improving their quality of life within the community.

Minimum Qualifications:

  • Fluency in both Spanish and English, with excellent verbal and written communication skills in both languages.
  • Bachelor's degree or higher in social work, human services, or related field preferred.
  • Experience working with diverse populations, particularly Spanish-speaking communities, in a social service or case management capacity.
  • Strong organizational skills with the ability to manage multiple cases and maintain detailed records.
  • Basic proficiency with computer applications including Electronic Health Records software and Microsoft Office Suite.

Preferred Qualifications:

  • Bachelor’s degree in social work, psychology, counseling, or a related human services field.
  • Certification or training in case management or paraprofessional services.
  • Experience working within community-based organizations or government agencies serving underserved populations.
  • Knowledge of local social service systems, healthcare resources, and legal rights relevant to clients.
  • Familiarity with trauma-informed care practices and culturally responsive service delivery.

Responsibilities:

  • Conduct thorough assessments of clients’ social, emotional, and economic needs while ensuring cultural sensitivity and language accessibility.
  • Develop, implement, and monitor individualized service plans in collaboration with clients and relevant stakeholders.
  • Provide ongoing case management support, including crisis intervention, advocacy, and referrals to appropriate community resources and services.
  • Maintain accurate and timely documentation of client interactions, progress, and outcomes in compliance with organizational and regulatory standards.
  • Facilitate communication between clients, families, service providers, and community organizations to ensure coordinated care and support.

Skills:

The required bilingual communication skills are essential for building rapport and effectively engaging with Spanish-speaking clients, ensuring their needs are accurately understood and addressed. Organizational and documentation skills are used daily to track client progress and maintain compliance with program requirements. The ability to coordinate with multiple service providers and advocate on behalf of clients requires strong interpersonal and problem-solving skills. Preferred skills such as knowledge of trauma-informed care and local resources enhance the quality of support provided and improve client outcomes. Proficiency with case management software and technology facilitates efficient case tracking and reporting, enabling the Case Manager/Paraprofessional to manage their workload effectively.
