About This Position
Position SummaryWe are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) or Medical Assistant (MA) to provide Principal Care Management (PCM) services in either a fully remote or onsite clinic setting. This role focuses on supporting patients with a single high-risk chronic condition through consistent, proactive engagement and care coordination.Working under the supervision of a physician or qualified healthcare provider, you will play a key role in improving patient outcomes by monitoring health status, reinforcing care plans, and ensuring continuity of care. This is an ideal opportunity for a clinician who enjoys patient communication, chronic care support, and the flexibility to work either remotely or in a clinic environment.Key ResponsibilitiesEnroll eligible patients into the Principal Care Management (PCM) programMaintain ongoing patient engagement and participation in PCM servicesConduct regular patient outreach via phone and secure messagingServe as a consistent point of contact for assigned patientsAssess patient-reported symptoms and medication adherenceIdentify and escalate significant changes in patient condition to the care teamReinforce and support implementation of individualized care plansProvide patient education to promote self-management and treatment adherenceCoordinate care services, including specialist referrals, diagnostic testing, and follow-upsAccurately document all patient interactions in the electronic medical record (EMR)Track and report time spent on PCM activities in compliance with program requirementsQualificationsActive, unrestricted LPN license or Medical Assistant certification in Georgia or a valid compact licensePrior experience in care coordination, chronic care management, or remote patient care preferredProficiency with Electronic Medical Records (EMR) systems and Microsoft OfficeStrong verbal communication skills, especially via phone-based patient interactionsExcellent organizational skills and attention to detailAbility to work independently while maintaining consistent productivity and documentation standardsCompensation & BenefitsW-2 employee positionStarting pay: $22/hour and up, based on experienceHealth insurance: Company covers 50% of premiums for employees and dependents401(k): Employer match up to 3%Paid federal holidaysOpportunity for growth within a rapidly expanding care management programWork EnvironmentFlexible work setting: Choose between 100% remote or onsite at the clinicRemote employees must have a reliable internet connection and a private, HIPAA-compliant workspaceOnsite employees will work in a clinical office environmentFlexible and adaptable to evolving workflows and program requirements
