About This Position

Description:

Required hours:

6:00 AM – 3:00 PM)– Monday through Friday

MAIN FUNCTION:

Provide compassionate care and assistance to individual(s) with intellectual and developmental disabilities including assistance with day to day living activities, personal hygiene, and vocational skill building activities.

WORK ENVIRONMENT:

  • Supports are provided in the home and in the community. May have direct contact with individuals who display unpredictable and challenging behaviors.

Requirements

Provides supports to individual(s) by assisting in the development and implementation of all support plans. Assisting individual(s) in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and supporting individual(s) to be independent in activities of daily living.

Uphold individual(s) supported rights to a healthy and mature environment for living, learning and employment.

Keep individual(s) supported safe from harm. Ensure individual(s) rights are respected and are treated with dignity.

Report on individual’s status via daily notes. Complete appropriate documentation concerning care of person supported. Areas to report should include, but are not be limited to Illness/injury Social conversation employment (if applicable) Abuse/neglect Materials Unusual Incident

Assists individual(s) with maintaining a clean, safe, and orderly home by following safety and infection control procedures. Comply with proper fire procedures, drill, and other emergency protocol as dictated by agency and/or state guidelines. Maintains the property of the agency and of the individual(s) supported.

Maintains and expands skills and knowledge relevant to providing high quality support and services. Obtains and maintains all required certifications and trainings and applies the information to individual(s) served. Attend all training outlined by the DIDD, state and agency guidelines.

Assists individual(s) supported with healthcare visits, medications, and implementation of all care plans.

Networks with coworkers, the community and other professionals to find better ways to include individual(s) supported in enriching life activities.

WORK ETHICS

  • Maintain good attendance record, including punctuality and leave request procedures.
  • Provides emergency coverage when requested by management.
  • Caring Attitude; promote good work habits; assist new employees.
  • Complete all duties and required documentation set forth by NIA policy/procedures/guideline and DIDD compliance.
  • Maintain clean and neat appearance with appropriate attire for accompanying person supported to various activities.
  • Model professional conduct and ethical behavior.
  • Maintains confidentiality of records/information according to HIPAA, State, Federal laws, and guidelines.

PHYSICAL AND MENTAL DEMANDS:

  • Must be able to move or lift a minimum of 25 pounds. The employee may occasionally lift and/or move up to 50 pounds with assistance from a second person or mechanical device.
  • Frequently bend, stoop, kneel, crawl, walk, and respond to challenging behaviors with prescribed techniques.
  • Must be able to work outside of normal work hours including nights, holidays and weekends as needed to meet Agency and position demands.
  • May involve sitting for brief periods of time.
