Physical Therapist - HH at Powers Health – Hobart, Indiana

The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates and provides physical therapy services to patients as prescribed by the referring physician. The PT develops plans that meet the functional, physical and educational needs of the homecare patient.

EDUCATION/EXPERIENCE:

1. Graduate of an accredited Physical Therapy curriculum with a minimum of a Bachelor of Science degree or graduate of an accredited college/university with a related major and completion of a certificate program in Physical Therapy.
2. State of Indiana Physical Therapy License required, or eligible for state licensure within 180 days.
3. Valid driver’s license and proof of auto insurance required.
4. Current CPR certification required.
5. Previous home health experience in direct patient care preferred.
