Responsibilities

The Lifestyle Medicine Dietitian is responsible for supporting patients along their whole person healthcare journey to help each patient achieve optimal health in collaboration with Kettering Health’s Lifestyle Medicine clinical team. This allied health professional will readily and proactively demonstrate and link the role of diet with health and well-being in alignment with Kettering Health’s Lifestyle Medicine curriculum. Through the microbiome and the gut-brain axis, diet is inextricably linked to the pillars of lifestyle medicine as its own pillar, and through its impact and influence on the other pillars: optimal nutrition, physical activity, stress management, restorative sleep, social connectedness, avoidance of risky behaviors and spiritual health. Connecting with patients by meeting each person where he or she is without judgement is one of the key tenets and foundational to assisting each patient. Providing support as patients navigate healthful behaviors, ensuring accountability, and using evidence-based tools and strategies are other core tenets. With the myriad of communication channels, the LMD will recognize the value of understanding each patient’s preference and be adept with multi-modal, synchronous and asynchronous communication. While the LMD needs to be comfortable with being autonomous, collaboration with the extended Lifestyle Medicine team is essential. The LMD will provide analytics that demonstrate measurable outcomes and create patient portfolios to help the patients map their progress as well as comprehensive reports with anonymized aggregate data to assess the lifestyle medicine program. The Lifestyle Medicine Dietitian will meet with patients one-on-one or in shared medical appointments to facilitate and inspire positive lifestyle changes related to the role of food as health and the biosocial elements of food on optimal health. Performs other duties as assigned.

Education:

· Minimum Bachelor’s Degree in Nutrition, Dietetics, Health Education, Community Health or equivalent

· Preferred Master’s Degree

Work Experience:

· Minimum Two (2) years clinical experience or equivalent

· Preferred Progressive healthcare leadership experience with demonstrated success

Essential Functions:

· Conduct initial dietary consult, utilize health history, and biometrics to establish baseline information to develop a tailored holistic nutritional plan, partnering with the Lifestyle Medicine Team, for the comprehensive lifestyle medicine plan, for each patient

· Work with Lifestyle Medicine team to support the implementation of each patient’s health plan

· Support patients in optimal nutrition goal development and create record of goals for each patient

· Help each patient meet or exceed their optimal nutrition goals

· Track the progress of each patient and the collective progress of the Lifestyle Medicine patient population

· Pursue knowledge to stay abreast of current health and wellness research and evidence-backed guidelines and initiatives to help support the transformation of patient’s lives and help each patient achieve their optimal potential

· Educate patients and families on optimal nutrition elements of lifestyle medicine

· Communicate with third party payors regarding issues of medical necessity and benefits coverage for patients seeking assistance with dietary lifestyle changes to improve their health and well-being.

· Organize and lead shared medical appointments utilizing Kettering Health’s lifestyle medicine curriculum

· Utilize Epic for clinical management of patients in the Lifestyle Medicine program

· Collaborate with the medical providers, health coach, exercise physiologist, chaplain and other health professionals to provide support for the patient and family

· Provide in-person, telephonic or video consultation and follow-up along with mychart communication

· Participate in the development and execution of quality assessment and performance improvement programs and projects

· Document appropriately to ensure that Kettering Health remains compliant with program delivery, billing and coding.

· Work with Research team to participate in research projects that support the Lifestyle Medicine Program and collect and analyze data for ongoing clinical research projects

· Incorporate research findings into the Lifestyle Medicine program

· Ensure Lifestyle Medicine Health Coaches are up to date on best practices

· Provide consultation to Kettering Health Care Professionals who provide care to patients seeking Lifestyle Medicine support/services.

· Create an environment that facilitates and supports the process of change for each patient and believe in the ability of each patient to make positive changes

· Recognize and value the individuality of each patient and understand the tools and strategies that best support each patient on his/her healthcare journey which includes helping patient understand their motivation for change

· Utilize the Kettering Health Lifestyle Medicine Curriculum as the foundation of lifestyle medicine information

Required License:

· RDN / RD or equivalent or CNS or equivalent or IFNCP or equivalent Conferring Organization

· Registered Dietitian by the Commission on Dietetic Registration

· Certified Dietitian in the State of Ohio

· Commission on Dietetic Registration

· Board for Certification of Nutrition Specialists

· Integrative and Functional Nutrition Academy Preferred

· Certified, American College Lifestyle Medicine

· Lifestyle Coach, certified through program approved by CDC within six (6) months of hire

· Certified, Diabetes Care and Education Specialist (CDCES)

· Board Certified-Advanced Diabetes Management (BC-ADM)

· Certified, Interdisciplinary Obesity and Weight Management by the Commission on Dietetic



Qualifications

One to two years’ clinical nutrition experience in acute or long-term care, with emphasis on nutrition support, diabetes or cardiac medical nutrition therapy preferred
