Position Summary
The Registered Nurse (RN) – Clinical Manager & Intake Support is responsible for providing clinical oversight, conducting supervisory visits, and ensuring high-quality patient care, while supporting the intake process to facilitate timely and appropriate patient admissions.
This role works collaboratively with the Intake Coordinator to ensure clinical readiness for new patients, continuity of care, and compliance with regulatory standards.
Supervision
Works under the supervision of the Director of Nursing, Clinical Nurse Manager, and Intake Service Manager.
Education
· Associate’s degree in nursing required; Bachelor of Science in Nursing (BSN) preferred.
· Active Registered Nurse (RN) license in good standing with the Kansas State Board of Nursing.
Experience
· Minimum of two (2) years of RN experience required.
· Five (5) years of RN experience preferred.
· Home health or community-based care experience preferred.

Job Duties & Responsibilities
I. Clinical Supervisory Responsibilities
· Provide initial patient assessments and evaluations, completing visits and documentation within 48 hours of assignment.
· Conduct routine patient supervisory visits, assessing patient condition and responding to changes in status. Documentation must be completed on the same day as the visit.
· Identify changes in patient condition and initiate appropriate referrals to physicians or healthcare partners. Ensure timely follow-up and documentation.
· Provide non-routine visits as needed to address changes in condition, safety concerns, or caregiver performance issues.
· Conduct supervisory visits of Home Health Aides and Patient Care Workers to ensure patient needs are met and the plan of care is followed.
· Educate patients and families on proper home health care strategies and procedures.
· Provide caregiver training and education on specialized care needs, including but not limited to:
o Catheter care
o Hoyer lift transfers
o Medication administration
o Diabetic care
· Maintain effective communication and coordination with physicians, caregivers, patients, families, and agency staff.
· Demonstrate clinical leadership, professionalism, and accountability in all patient care activities.
II. Intake Support Responsibilities
· Collaborate with the Intake Coordinator to support timely and efficient patient admissions.
· Assist in reviewing referral information for clinical appropriateness and service needs.
· Support the collection and verification of required clinical documentation, including physician orders as directed.
· Communicate with patients, families, and referral sources as needed to facilitate the intake process.
· Provide clinical input to assist in determining patient care needs and service planning.
· Coordinate with internal staff to support timely scheduling and initiation of services.
· Ensure all intake-related documentation is accurate, timely, and compliant with agency policies and state regulations.
· Identify and communicate any clinical concerns or barriers to admission to the Intake Coordinator and/or leadership.
Administrative Responsibilities
· Adhere to all agency policies, procedures, and Kansas state regulations.
· Ensure implementation of patient rights and responsibilities.
· Maintain confidentiality and compliance with HIPAA regulations.
· Complete required documentation accurately and within required time frames.

Performance evaluations will occur within six (6) months of employment and annually thereafter.
