About This Position
Coastal Kids Home Care is a pediatric licensed home health agency. Come develop excellent skills, feel appreciated every day, you don't work short-staffed or feel like you've been run over by a truck at the end of a shift - 1:1 training in the home with an experienced nurse, safe and paid at full rate. SAN JOSE 95148 – 5-year-old little boy with a history of medulloblastoma on maintenance therapy with a prolonged hospitalization due to neurosurgical complications. VP shunt in place, GJ dependence, and chronic resp failure. Nurses responsibilities include providing all ADLs, administering medications and feeding via GJ tube as ordered and providing all pulmonary care as ordered. Needing Days/NOCs. Nurses would work 3 - 12 shifts per week.
SAN JOSE 95118 - RN preferred but parents are open to an experienced LVN. 9-year-old little boy with chronic obstructive pulmonary disease that is gtube dependent. Nurses responsibilities include providing all ADLs, administering medications and feedings as ordered along with any pulmonary needs as ordered. This is a sweet little boy who just wants to be cuddled! Needing Sun-Thursday Noc; Sat & Sunday PMs; RN only
FREMONT 94536 - 25-year-old young man with chronic respiratory failure that is vent, trach and gtube dependent. Nurses responsibilities include providing all ADLs, administering medications and feedings via gtube and providing all pulmonary care including trach suctioning, breathing treatments and scheduled trach changes. Needing Fridays 4pm-12am; Saturday/Sunday 3pm-11pm Fridays 4pm to 12am; Saturdays 3pm-11pm; Sunday 3pm-11pm
SANTA CLARA 95122 - 9-year-old boy with a diagnosis of chronic hypoxic respiratory failure, ineffective airway clearance and acute flaccid myelitis who is vent/trach and gtube dependent. Nurses responsibilities include providing all ADLs, administering all medications and feedings via gtube and providing any and all pulmonary care including trach suctioning, breathing treatments, and scheduled trach changes. Weekends 8am-4pm
SAN JOSE 95122 – 8-year-old little boy with a primary diagnosis of spinal muscular atrophy, g-tube dependent along with chronic respiratory failure requiring nightly Bipap. Nurses responsibilities include providing all ADLs, administering feeds and meds via gtube or PO as ordered and providing all pulmonary care including oral and nasal suctioning, vest treatments and breathing treatments as ordered. Nurse will attend school with patient daily. Needing M-F 8am-4pm
SAN JOSE 95136 - 13-year-old girl with a diagnosis Dravet Syndrome and history of seizures that is gtube dependent. Nurses responsibilities include providing all ADLs, administering medications and feedings as ordered and providing any pulmonary care as ordered. Saturday 8am-4pm
SANTA CLARA 95050 - 15-month-old sweet little girl diagnosed with congenital ventriculomegaly, gross mother developmental delay, GERD, hypotonia, sleep apnea, acute on chronic hypoxemic respiratory failure, anemia and is trach/vent, G-tube dependent. Nurses responsibilities include providing all ADLs, administering all medications and feedings via gtube as ordered and providing all pulmonary care that includes trach suctioning, oral suctioning, providing scheduled and PRN breathing treatments and performing scheduled and PRN trach changes. Needing M-F Noc 10pm-6am; Weekends Noc 10pm-8am
Qualifications
- LVN license in the state of California
- Proficiency in basic life support and patient care
- Ability to assess and provide nursing care to pediatric patients
- Knowledge of HIPAA regulations and patient privacy
- Willingness to work in home health settings
Responsibilities
- Provide compassionate and skilled nursing care to pediatric patients in a home setting, ensuring adherence to HIPAA regulations.
- Conduct patient assessments and monitor vital signs, administering basic life support as needed.
- Manage tube feeding, and medication administration, while maintaining accurate electronic health records.
- Collaborate with families and healthcare teams to develop and implement individualized care plans.
- Educate patients and caregivers on health management and safety practices.

The pay range for this role is:
39 - 42 USD per hour(Santa Clara, Ca)
