Job Summary:
-
Licensed Practical Nurse at CalvertHealth will work with the value-based care staff and clinical staff to identify, schedule, coordinate and perform Annual Wellness Visits for CalvertHealth’s Centers for Medicare & Medicaid Services.

Education:
-
Completion of an accredited LPN Program and LPN - State Licensure
-

Registration/Certification/Licensure:
-
- Current Maryland State LPN License in good standing
- CPR, BLS Certification
- NRP Certification within 6 months
-

Experience:
-
- 1 - 2 years of experience as an LPN preferred
- Experience in primary care, geriatrics, or chronic disease management preferred
- Familiarity with Medicare wellness visit guidelines and documentation requirements

Other Requirements:
-
Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements.

Proficiency with EHR systems and clinical documentation

Strong communication and patient engagement skills
-
Attention to detail and ability to follow clinical protocols

The Licensed Practical Nurse (LPN) contributes to the provision of high-quality, cost-effective, healthcare as a provider of direct and indirect patient care.
