Affirmed Home Care is currently seeking Per Diem Licensed Practical Nurses (LPNs) with pediatric tracheostomy and ventilator experience to join our compassionate, high-performing team throughout Nassau/Suffolk County. As part of our continued growth, we are actively building and expanding a dedicated NEW Pediatric Division on Long Island, and this is an exciting opportunity to be part of something new from the ground up.

As an Affirmed LPN, you will deliver individualized, one-on-one skilled nursing care to pediatric clients with complex medical needs in the comfort of their homes. This role is ideal for experienced nurses who are confident in trach and vent management and are looking for meaningful, patient-centered work. We offer day shifts, overnights, and per-visit opportunities, allowing for flexibility while making a true impact.

• Minimum 1 year of recent acute care pediatric LPN experience

• Pediatric tracheostomy and ventilator care experience required

• Active New York State LPN license

• Current PALS certification

• Eligibility to work in the U.S.

• Valid driver’s license (preferred)

• Physical exam within the past 12 months

• PPD or QuantiFERON within 1 year, or chest X-ray within the past 5 years

• MMR immunization within the last 10 years
